Important Update - Invesco Fair Fund Distribution Process
As a courtesy, this update is to inform you that the distribution process is anticipated to close soon and you should act immediately if you have a payment(s) you wish to have re-issued and/or negotiate.
- September 17, 2010: Last date a check will be re-issued or replaced
To ensure your check re-issuance request is completed on time please submit all necessary documentation immediately. Checks cannot be re-issued for documentation that is received after September 17th or where the information provided is incomplete.
Distribution checks will not be re-issued after September 17, 2010.
- October 22, 2010: Last date a check will be honored for payment
If you are currently in possession of a check that does not require further changes please negotiate your payment immediately. The Fair Fund distribution account will continue to honor checks for payment, regardless of the void date on your check, through October 22, 2010.
Any check presented after October 22, 2010 will not be honored.
The Invesco Fair Fund, the Independent Distribution Consultant, Invesco and the Fund Administrator are not responsible for any fees you may incur from your bank.

Tax Information

Reissue Forms
These forms are to be used should you wish to have your retirement check made payable to yourself. In the event of a deceased situation, the beneficiary form would apply and will need to be completed by each beneficiary of record.
Reissue IRA Distribution Form:
IFG Reissue IRA Distribution Form
For use by individuals who received a check written to their IRA and no longer maintain an IRA.
Beneficiary Reissue IRA Distribution Form:
IFG - Beneficiary Reissue IRA Distribution Form
For use by beneficiaries or guardians who received a check written to an individual’s IRA and it should be reissued to the beneficiary or guardian.
Reissue Non-IRA Distribution Form:
IFG Reissue Non-IRA Distribution Form
For use by individuals who received a check as part of a Retirement Plan or a Defined Benefit Plan and the Plan or employer no longer exists.
Beneficiary Non-IRA Distribution Form:
IFG Beneficiary Reissue Non-IRA Distribution Form
For use by beneficiaries or guardians who received a check written to an individual as part of a Retirement or a Defined Benefit Plan and it should be reissued to the beneficiary or guardian.
Abandoned Plan Affidavit:
IFG - Abandoned Plan Affidavit Form
This form must be submitted when an individual requests to have an ERISA Plan (401(k), 403(b)) check reissued to their own name. This form will be submitted with the letter of instruction, the correct reissue form (see above), and the IRS Form 5500, which can be obtained from the www.freeerisa.com website.
